About Your Visit
We request that new patients arrive 15 minutes prior to their scheduled appointments. All patients should present their current insurance cards, registration forms (found below), and a photo IDs when checking in for their appointments.
If you need to cancel your appointment, it is highly preferred that you call at least 24 hours beforehand. Otherwise, please call the office as soon as possible. You can leave a message with our answering service if you call after hours.

Co-pays must be made before your scheduled appointment. If you miss more than one appointment without notifying our office, you will be charged a penalty.
